Please be advised that all contractor licenses expire one year from the date of issuance. It is the contractor’s responsibility to renew the license before its expiration and to provide updated insurance forms. If the license expires, a $40 late fee will be assessed. To renew the license before expiration, please submit the renewal form, new insurance documentation, and a $20 renewal fee.
Certificate of Insurance specifies liability coverage and names the District as additional insured, set at a minimum of $1,000,000.
